  1. Attendance and Cancellation Policies

     

    Thank you for choosing Speech-Language Associates.  I want to provide the best possible service to all of my patients. I will do my best to schedule appointments that meet your needs.  Regular attendance is important to your child’s success. I ask that you follow the attendance policies outlined below.

     

    Scheduled appointment lengths include the following (unless otherwise stated to you).

             • 30 minutes – This consists of 25 minutes contact time and any discussion about the home program plus 5 minutes to use on your child’s behalf for preparation, analysis of treatment activities, charting and planning for the next session.

             • 45 minutes – This consists of 40-minute contact time and any discussion about a home program plus 5 minutes to use on your child’s behalf for preparation, analysis, charting of treatment and planning for next session.

             • 60 minutes – This consists of 50 -55 minutes of contact time and any discussion about a home program plus 5 – 10 minutes to use on your child’s behalf for preparation, analysis and charting of treatment activities.

     

    1.      Cancellations: When possible, provide at least a 24-hour notice if an appointment will need to be cancelled. You can provide a cancellation notice either by e-mail (Chris@SLAssoc.com) or phone (303-926-4215).

     

    You need to cancel even if you are unable to provide a 24-hour notice. Please let me know about appointments that need to be cancelled as soon as possible.

     

    If a therapy session is cancelled by you, I will reschedule a make-up session if my schedule permits.

     

    2.      Missed Appointments:  If you cancel an appointment or not show up at your scheduled appointment five times during the course of your child’s treatment, I will put your services on hold until scheduling problems can be worked out.  The same will hold true for frequent cancellations without rescheduling.

     

    3.      Late for Appointments: If you arrive late to an appointment, therapy will still end at the regular time and you will be responsible for the full session fee.  If you are late for five or more sessions, we may put your services on hold until scheduling problems can be worked out.

     

    4.      Clinician Cancellations:  If I am not able to attend your appointment, you will be contacted as soon as possible.  Please be sure my office knows the best way to reach you.  Every effort will be made to reschedule you appointment in a timely manner.

If you are coming for an evaluation, it is helpful if the forms can filled out and sent to Speech-Language Associates, LLC, at least two days before your appointment so that we may prepare for your evaluation.
